Key Responsibilities: - Conduct comprehensive evaluations of patients' physical, neurological, respiratory, and cardiovascular function using formal and informal assessments.
- Develop individualized, goal-oriented treatment plans based on assessment data and patient tolerance.
- Document treatment regimes accurately and submit plans for physician approval, updating plans as patient conditions evolve.
- Coordinate treatment schedules with patients, families, and clinical staff, ensuring clear communication of treatment goals and progress.
- Educate patients, families, and caregivers about their conditions, treatment options, prognosis, and preventive strategies through effective teaching methods.
- Delegate responsibilities appropriately to Physical Therapist Assistants, aides, and students, providing supervision and guidance.
- Recommend and facilitate the ordering of adaptive equipment or support devices based on patient assessments.
- Ensure accurate and timely documentation for regulatory compliance and billing purposes.
- Participate in interdisciplinary team conferences, contributing to care planning and program development.
- Promote physical therapy services within the community and assist in public education initiatives.
- Act as a case manager for assigned patients, monitoring progress and adjusting treatment plans accordingly.

Qualifications: - Valid state licensure as a Physical Therapist.
- Prior outpatient therapy experience preferred but not required; new graduates considered.
- Demonstrated knowledge of physical therapy standards, legal and regulatory requirements.
- Strong communication skills—verbal and written—in English.
- Ability to work independently and as part of a team, with excellent organizational and time-management skills.
- Competency in Microsoft Windows and electronic documentation systems.
- Ability to lift/push/pull up to 50 lbs and perform physical tasks associated with patient care.
- Willingness to participate in OSHA training and follow safety protocols, including use of personal protective equipment.
Physical and Mental Job Demands: - Capable of performing duties in varying environments, including moving about the facility with or without assistive devices.
- Able to communicate effectively and handle stressful situations involving trauma, grief, or difficult patient interactions.
- Frequent postural changes, including sitting, standing, walking, and reaching.
