Physical Therapist - Home Health - (PT - HH)
StartDate: 10/20/2025 Available Shifts: 8 D/E/N Pay Rate: $2525.20 - $2766.97

PT- home health needed asapPlans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.Bachelor's Degree or equivalent (some certifications will require Bachelor?s): RequiredMaster's Degree: PreferredPhysical Therapy licensure in all applicable states of practice: RequiredCardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: RequiredValid Driver's License (DL) and must be at least 21 yrs of age or older: Required

Licenses: PT-CA
Certifications: BLS(Copy Needed)
SSN Required
DOB Required
References: 1 Reference in entire work history

Bachelor's Degree or equivalent (some certifications will require Bachelor's): Required
Valid Driver's License (DL) and must be at least 21 yrs of age or older: Required

Preferred Qualifications
Master's Degree: Preferred
