Physical Therapist in Camden, NJ

A leading home health program is seeking a Physical Therapist to provide in-home therapy services to elderly patients aimed at maintaining their independence and enhancing their quality of life. This role involves conducting patient evaluations, developing personalized treatment plans, and collaborating with a multidisciplinary care team in a supportive and patient-focused environment.

Key Responsibilities:• Perform thorough patient assessments and create individualized care plans.• Deliver therapeutic interventions to improve mobility, function, and quality of life.• Educate patients and caregivers on exercises and effective home care strategies.• Collaborate closely with physicians, nurses, and case managers to optimize patient outcomes.• Maintain accurate, timely, and compliant documentation of all patient interactions.

Qualifications:• Graduate of an accredited Physical Therapy program.• Active Physical Therapist license in New Jersey.• AHA Basic Life Support (BLS) certification.• Minimum of 2 years’ experience working with adult geriatric populations.• Proven experience in home health and senior care settings.• Strong communication, documentation, and critical thinking skills.• Reliable transportation for home visits within Mercer County and North Burlington County.

Preferred Qualifications:• Experience in pelvic floor therapy.• Completion of Herman & Wallace Level 1A training or equivalent.

Physical Demands & Work Environment:This position requires frequent standing, walking, and driving between patient homes, as well as occasional lifting up to 50 lbs., bending, and reaching.

Schedule:• Monday through Friday• 8:00 AM to 4:30 PM• No on-call duties or weekend work required
