StartDate: ASAP Available Shifts: 8 D Pay Rate: $2326.00 - $2397.00
2+ years of experience. 50-mile radius rule. No more than a 3-month work history gap in the last year. Must have previous Home Health experience. Average of 5-6 home visits per day, covering all surrounding areas of MCDowell county. EMR: Cerner Roadnotes. Can expect to do Home Health admisions, ROCs, recerts, functional assessments, supervising PTAs. One weekend of call every 4-6weeks required.Tell us about the unit(s): Number of Beds approximately 70 patients that are seen by any combination of disciplines Number of Staff 25 (including nursing, rehab, office, and administrative staff) Type of staff: Works with other PTs, PTAs, OT, RNs, CNAs, SW. This service is carried out where the patient calls home: personal home, ALFo Day Shift Office open 8-4:30 Monday-Friday (Nurse on call 24/7)o Night Shift - NA Patient Ratios 6 visit equivalents per day ( types of visits are weighted differently on a point system) Type of equipment EMR - CernerTypical hiring profileSkill Set Most recruited for: Home Health Must have: candidates without these skills will not be considered for the role. Experience in completing OASIS Assessments Home Health experiencePreferred or nice to have: candidates with these skills will be considered first. Some knowledge of new payment model coming 1-1-20 (PDGM) List typical procedures performed on unit(s): Comprehensive assessments/evaluations, treatment and care planning. Oversight of PTAs. Patient population: mainly geriatric; post-acute treatment of THA/TKA patients; treatment of patients with chronic illnesses and multiple comorbidities.Best personality Fit: Someone who is easy going, likes to meet people and doesnt mind visiting people in their own homes. Needs to be accepting of others and friendly. REQUIRED CERTIFICATIONS:BLS/CPR certification. Needs current NC license to practice Physical TherapyFloating Area(s): noneDress Code: Not required to wear scrubs. Business casual, closed toed shoes. Unit Guidelines/Policies: Please feel free to attach unit policies and guidelines. Will be discussed in interview. Weekends and On call(if any) Weekends only as needed, not as a routine expectation
