What You’ll Do: - Effectively screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders.
- Develop individualized plans of care, utilizing evidence-based assessments and interventions.
- Monitor patient progress, adjusting treatment plans to ensure optimal outcomes.
- Evaluate and document treatment effectiveness, making necessary modifications and referrals.
- Provide consultation and counseling to patients, families, caregivers, and interdisciplinary team members.
- Communicate regularly with supervisors and healthcare team regarding patient progress and barriers.
- Prepare comprehensive discharge summaries and maintain detailed, compliant documentation.
- Participate in meetings, trainings, and ongoing professional development.
- Support clinical supervision of CFYs or student Speech-Language Pathologists.
- Ensure adherence to ASHA and State practice standards, ethics, and licensure requirements.
Required Skills: - Valid state license and ASHA certification as a Speech-Language Pathologist.
- Proven experience in diagnosis and treatment of communication and swallowing disorders.
- Strong assessment, clinical decision-making, and documentation skills.
- Excellent interpersonal skills to effectively communicate with patients, families, and team members.
- Ability to work independently and as part of a multidisciplinary team.
Nice to Have Skills: - Experience working in long-term care or institutional settings.
- Bilingual proficiency or additional language skills.
- Knowledge of telehealth platforms and digital documentation systems.
Preferred Education and Experience: - Master’s degree in Speech-Language Pathology or equivalent.
- Prior experience in a clinical or long-term care environment preferred.
- Licensure and certification active and in good standing.
Other Requirements: - Ability to obtain necessary durable medical equipment (DME) and supplies.
- Willingness to support and supervise CFYs or student clinicians.
- Committed to practicing ethically and professionally per ASHA and state guidelines.
