This setting allows for meaningful, one-on-one relationships and the ability to see your interventions directly improve function, safety, and independence in the environment where patients live every day.You will be responsible for performing comprehensive physical therapy evaluations in the home, developing individualized plans of care, and delivering evidence-based treatments focused on strength, balance, mobility, endurance, and pain management. Typical conditions include post-surgical rehabilitation (such as joint replacement), neurologic conditions, cardiopulmonary disease, fall risk and balance disorders, chronic musculoskeletal conditions, and functional decline associated with aging or recent hospitalization.A typical day includes traveling to multiple patient homes within a defined territory, coordinating visits with patients and caregivers, and collaborating closely with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, social work, and home health aides. You will educate patients and families on home exercise programs, fall prevention strategies, safe use of assistive devices, and environmental modifications to support safety and independence. Patient visit volumes are designed to support thorough, high-quality care and appropriate travel time, with a focus on clinical outcomes and patient satisfaction.Schedules often allow for a degree of flexibility, with daytime hours and the opportunity to cluster visits geographically to limit drive time.
