Summary:
- Coordinate the Cancer Risk Evaluation Program (CREP) at Chester County Hospital, provide patient information and recruit for program, perform risk assessments and genetic counseling, facilitate research enrollment, provide community education and outreach in regards to cancer risk evaluation.
Responsibilities:
- Perform cancer risk assessments and genetic counseling in collaboration with physicians.
- Coordinate genetic counseling, genetic testing and genetic testing results visits. Make recommendations for patients and their family members regarding screening and management of cancer risks. Include physician in assessing appropriate genetic testing, cancer screening and management recommendations.
- Facilitate patients in understanding the importance of informing family members of risks and, when appropriate, help patient’s family members through the genetic counseling and testing process.
- Attend Penn CREP genetics conferences and present cases, contribute to discussion, and expand on cases when necessary based on recommendations. Follow up by providing new information and education to appropriate physicians and staff.
- Attend cancer genetics related seminars and meetings when appropriate. Report relevant material back to colleagues.
- Attend breast conference and makes recommendations for genetic counseling and screening for appropriate patients.
- Coordinate the day-to-day tasks for the Cancer Risk Evaluation Program (CREP), including:
- Triage patient care for prior and new referrals to CREP.
- Market CREP program through meetings with other health care providers, market through community events, and continue to update and develop marketing tools with the guidance of Oncology and Marketing Leadership- such as brochures, bookmarks and posters.
- Collaborates with representatives of testing labs to help resolve billing issues that patients experience.
- Maintains database of all patients seen and provides reports as requested for Oncology Leadership.
- Develop, plan, coordinate and participate in the promotion of cancer prevention, patient health education and community outreach programs, including:
- Give lectures when asked and available in local community and internally within the hospital.
- Promote importance of family history in health care decisions
- Work with the Oncology Administrative Director and other internal team members, to coordinate and participate in health screening events and community events.
- With the team above, evaluate community education programs and improve as necessary.
- Participate in research activities including the following: Work with Oncology Research team to recruit and screen patients for participation in studies.
- Provide staff education related to cancer prevention and research protocols.
- Educate staff on updates in CREP as they relate to other provider’s role in assessing family history, providing patient education and referring patients
- Attend meetings relative to job function, which may include Cancer Committee and Breast Health Task Force Meetings. Provide reports when requested.
- Assist with fundraising and development for CREP program as appropriate.
- Demonstrate the skills necessary to provide care appropriate to the age of the patients served. Including:
- Provides instruction appropriate to the age of the learner(s).
- Interacts with the patient in a manner that preserves patient dignity, privacy and meets psychosocial needs of the patient.
- Integrates principles of growth and development into practice and identifies each patient’s requirements relative to his/her age specific needs.
- Documents problems and interventions for identified patient needs and outcomes.
- Documents that patient has received and acknowledged the HIPAA Notice of Privacy Practices.
- Assists with scheduling of patient appointments for appropriate treatment, time etc.
- Documents prescribed treatments, therapies, assessments and interventions.
Credentials:
- Genetic Counselor - National Board Certification (Required)
- Genetic Counselor - License (Required)
- Accepts individuals on a provisional basis with the expectation to take boards, pass, and obtain PA licensure by end of 180-day probationary period.
Education or Equivalent Experience:
- Master of Arts or Science (Required)
- 1 year experience in genetic counseling preferred.
- Maintains on-going continuing education units required by ABGC.
