Responsibilities:
- Screen, evaluate, and rehabilitate patients using standardized assessments and evidence-based practices.
- Develop and implement individualized plans of care aligned with regulatory and clinical standards.
- Monitor patient progress, adjusting treatments as necessary and documenting all activities in the electronic records system.
- Evaluate treatment outcomes and modify services or make referrals accordingly.
- Provide consultation and counseling to patients, families, caregivers, and healthcare providers regarding speech disorders.
- Communicate effectively with supervisors and interdisciplinary team members about patient status and treatment plans.
- Deliver comprehensive discharge summaries adhering to all regulatory requirements.
- Participate in patient care conferences, utilization reviews, rehabilitation meetings, and mandatory trainings.
- Obtain necessary durable medical equipment or supplies in compliance with clinical standards.
- Supervise and support Clinical Fellowship Year (CFY) or student Speech-Language Pathologists.
- Adhere to ASHA and State licensure requirements, codes of conduct, ethics, and practice acts.
Qualifications and Desired Experience:
- Active ASHA certification and State licensure as a Speech-Language Pathologist.
- Proven experience in hospital-based therapy services, particularly in communication, cognitive, or swallowing disorders.
- Familiarity with documentation systems and regulatory compliance.
- Strong clinical reasoning, patient management, and interprofessional communication skills.
- Ability to supervise and mentor junior clinicians or students.
