Physical Therapist - Home Health - (PT - HH)

Travel PT Home Health jobs in Southboro, MA are available for a 13-week assignment with Monday to Friday 8-hour days and 40 hours per week. You will assess patient mobility, develop individualized treatment plans, implement therapeutic interventions, and educate patients and caregivers on safety and home exercise programs. Daily responsibilities include 6-7 visits, documentation in Homecare Homebase, and collaboration with an interdisciplinary team[1].
