What You'll Do: - Effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs, developing individualized plans of care that adhere to all regulatory and clinical standards.
- Utilize standardized assessments and evidence-based practices to support clinical decision-making.
- Administer various therapeutic procedures, including manual techniques, ambulation, exercises, modalities, supportive and assistive devices, and wound care.
- Monitor patient progress, adjust treatments accordingly, and document outcomes in compliance with regulatory and clinical requirements.
- Provide consultation and counseling to patients, families, caregivers, and interdisciplinary team members.
- Communicate regularly with supervisors and team members regarding patient progress, barriers, and treatment plans.
- Prepare comprehensive discharge summaries and maintain detailed patient records in electronic documentation systems.
- Participate in meetings such as patient care conferences, utilization reviews, and team trainings.
- Support supervision of physical therapist assistants, rehabilitation aides, and students when applicable.
Required Skills: - Valid state license to practice as a Physical Therapist.
- Proven experience in rehabilitation, wound care, or related settings.
- Strong assessment, treatment planning, and documentation skills.
- Excellent communication and teamwork abilities.
- Knowledge of regulatory standards and evidence-based practices.
Nice to Have Skills: - Experience with advanced modalities and assistive devices.
- Certification in wound care or specialized therapeutic techniques.
- Prior supervisory or mentorship experience.
Preferred Education & Experience: - Bachelor's or Doctorate degree in Physical Therapy from an accredited program.
- At least 2 years of clinical experience preferred.
Other Requirements: - Ability to work weekdays during the day shift.
- Willingness to comply with all employer policies, including licensure, certifications, and safety standards.
- Capable of obtaining any required DME or supplies.
This position offers an hourly pay rate of $58.96 and is designed to be an engaging and rewarding experience.
