Key Responsibilities:
- Effectively screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders.
- Develop and implement personalized care plans aligned with all regulatory and clinical standards.
- Utilize standardized assessments and evidence-based practices to guide treatment.
- Monitor patient progress, adjusting interventions as necessary, and document outcomes comprehensively.
- Provide consultation and counseling to patients, families, caregivers, and interdisciplinary team members.
- Communicate regularly with supervisors and team members regarding patient status, barriers, and treatment strategies.
- Prepare detailed discharge summaries in compliance with regulatory requirements.
- Maintain accurate documentation of clinical services, including treatment minutes.
- Participate in meetings, trainings, and conferences relevant to clinical practice.
- Support supervision of CFYs or student SLPs.
- Obtain necessary DME or supplies per clinical standards.
- Adhere strictly to ASHA and state licensure regulations, codes of conduct, and ethical standards.
Required Skills:
- Active state licensure and ASHA certification.
- Proven experience in speech, language, cognitive, or swallowing disorder treatment.
- Strong assessment and clinical documentation skills.
- Excellent communication and collaboration abilities.
- Ability to work independently and as part of a multidisciplinary team.
Nice to Have Skills:
- Experience working in long-term care or similar healthcare settings.
- Bilingual proficiency (additional languages) is advantageous.
- Familiarity with electronic health record systems.
Preferred Education and Experience:
- Master’s degree or higher in Speech-Language Pathology.
- Minimum of one year of clinical experience in a healthcare setting.
Other Requirements:
- Ability to work daytime hours in a long-term care environment.
- Willingness to participate in ongoing training and team meetings.
- Valid driver’s license and ability to obtain any necessary supplies or equipment.
