Job Summary and Qualifications
Responsible for developing and coordinating initiatives directed at the appropriate use of antimicrobials within the organization. This includes developing, implementing and maintaining antimicrobial stewardship program, daily patient care monitoring and continuous performance improvement. Responsible for interdisciplinary education regarding antimicrobials and AMP programs.
Pharmacist will double check and continue to review neonatal and pediatric doses of medications and IV fluids to ensure that all weight- based doses are appropriate. This includes the ability to appropriately dose & assess medications that require pharmacokinetic adjustments, as well as quickly assess the literature for atypical medications & their doses Pharmacist should be able to review and evaluate NICU TPN orders for appropriateness and be able to write a NICU TPN if requested to by physician. Pharmacist should also be able to write a pediatric TPN in conjunction with the physician’s specifications (ex: fluid rate) when consulted
- Participates in daily rounds in NICU and PICU
- Ensure NICU/Pediatric/PICU departments medication-related policies and procedures are written, revised, up-to-date, and readily available for review. Work in conjunction with the multi- disciplinary teams when developing or revising a policy
- Pharmacy yearly reviews palivizumab (Synagis®) guidelines and administration time lines to help assess orders for appropriateness
- Readily available via imobile for questions during pediatric pharmacy hours
- Pharmacist will review Vigilance for clinically relevant pediatric alerts and contact the prescriber if needed.
- Pharmacist will double check and continue to review neonatal and pediatric doses of medications and IV fluids to ensure that all weight- based doses are appropriate. - Reviews NICU patient’s vaccination status and help keep patients up-to-date on required vaccinations
- Responds to all pediatric codes during pharmacy open hours; when responding to codes the pharmacist will be the primary person who is drawing up medications
- Participates in the Pharmacy and Therapeutics Committee and attends meetings when needed
- Participates in the Department of Pediatrics Committee and attends meetings
- Participates and attends NICU interdisciplinary Unit Practice Council Meetings
- Manages pharmacy staff development, competency and training in NICU/PICU/PEDS programs in collaboration with the Pharmacy Clinical Manager.
- Provides the patient/parent education of newly diagnosed type 1 diabetic patients – this includes providing type 1 diabetes educational material to the parents/patient and completing the appropriate sections of the “Type 1 Diabetes Checklist” that can be found on forms on demand.
- Provides medication discharge instructions for NICU parents when NICU patients are getting close to being discharged. Also, help facilitate the obtaining of outpatient medications that NICU parents must obtain prior to baby being discharged from the NICU. Nurse/Pharmacist must verify that this outpatient medication has the correct labeling on the bottle. This includes verifying the name/strength of the medication, dosing instructions, and storage instructions.
- Provide pediatric medication safety initiatives/information to nursing/medical staff via verbal communication or pharmacy newsletter
- Provides education and training to pharmacy students.
- Performs pharmacist duties as assigned.
- Demonstrates the ability to create, initiate, and integrate new concepts and practices to improve patient care and/or employee satisfaction.

QUALIFICATIONS:
Doctor of Pharmacy or Master’s degree required. Pharmacy residency training or Board Certification with added qualifications in pediatrics is preferred.
Dedicated Pediatric Lead pharmacist with the equivalent of > 5 years’ experience providing pharmaceutical care for neonates and/or pediatric patients. Or PGY1 Residency with emphasis on pediatrics within a pediatric hospital Or PGY2 Residency with Pediatric specialty
Board Certified Pediatrics. Maintain a knowledge of NALS in regards to appropriate medication, dose, and administration
Registered pharmacist licensed in the State of Florida.
