With a low cost of living and a slower pace than urban centers, Albany gives you room to breathe, explore the outdoors, and enjoy a balanced lifestyle.As a Home Health Physical Therapist, you will provide one-on-one, evidence-based care to adult and geriatric patients in their homes in Albany and surrounding rural communities. You will conduct comprehensive evaluations, develop individualized treatment plans, and deliver interventions designed to improve functional mobility, maximize safety, and support patients in achieving their highest level of independence at home.Typical daily responsibilities include performing in-home assessments, creating and progressing home exercise programs, providing gait and balance training, educating patients and caregivers on safe transfers and use of assistive devices, and coordinating care with nurses, occupational therapists, speech therapists, social workers, and primary care providers. You will also play a key role in fall prevention, post-operative rehabilitation, chronic disease management support, and discharge planning from higher levels of care back into the home environment.Your schedule will generally follow daytime hours on weekdays, with potential for some flexibility based on patient needs and agency structure. Caseloads are designed to allow for reasonable drive times in and around Albany, with visits scheduled to minimize travel where possible. You can expect a mix of start-of-care visits, routine follow-ups, recertifications, and discharge visits, with time allocated for same-day electronic documentation.Working in this home health role provides a high level of autonomy and professional satisfaction, as you will see firsthand how your interventions improve patients safety and independence where it matters mosttheir own homes.
