You will perform comprehensive evaluations, develop and implement individualized care plans, and provide skilled interventions to promote mobility, safety, and independence for adult and geriatric patients. Typical cases may include post-acute rehabilitation after hospitalization or surgery, fall risk and balance training, strengthening and endurance programs, cardiopulmonary conditioning, and functional training for daily activities in the home. You will collaborate closely with nurses and other rehab professionals to coordinate care, update plans based on patient progress, and communicate with referring providers.A typical day includes traveling throughout the Marion area to see a scheduled caseload of home visits, documenting care in an electronic medical record, and adjusting treatment plans based on patient response and goals. Patient visits are generally scheduled to allow adequate time for quality, hands-on care and thorough education. You can expect a primary focus on adult and geriatric patients, often with comorbid conditions, where your clinical judgment and creativity in the home environment will be highly valued. Work schedules often include daytime hours with some flexibility based on patient needs, and may include occasional weekend or on-call rotations depending on agency requirements.This position is well-suited for a Physical Therapist who enjoys autonomy, variety, and the opportunity to see patients in their own homes, where functional progress is most evident.
