ResponsibilitiesAs a social worker you will provide comprehensive support across various medical social work areas, including hospital inpatient and transitional care. This position serves as a critical resource within multidisciplinary teams, focusing on patient-centered care and collaboration to ensure optimal discharge planning and coordination of services.
Key Responsibilities
- Collaborate with multidisciplinary teams to assess and support the needs of the patients and their families
- Coordinate patient discharge planning, ensuring all necessary resources and support systems are in place
- Conduct biopsychosocial assessments to evaluate patients' needs and appropriate interventions
- Engage patients and families in discharge planning processes
- Facilitate communication and coordination with community partners and resources to support ongoing patient care
*This position is not eligible for visa sponsorship with the exception of the TN visa classification; Also, Mayo Clinic DOES NOT participate in the F-1 STEM OPT extension program.
QualificationsBachelor's degree in social work (BSW) or master's degree in social work (MSW) is required.
Licensure/Certification
Licensed Certified Social Worker (CSW), Licensed Advanced Practice Social Worker (APSW), Licensed Clinical Social Worker (LCSW) by the Wisconsin Department of Safety and Professional Services is required.
*APSW personnel, following 3,000 hours of clinical social work practice or 4,500 hours dependent upon education credits within the MSW program including at least 1,000 hours or 1,500 hours respectively of face-to-face contact supervised by a person meeting board requirements, must apply for licensure at the LCSW level. A condition of employment at the APSW level is the ability to practice clinically leading to LCSW attainment.

- Previous experience in a medical or healthcare setting is preferred

- Strong communication and interpersonal skills to effectively engage with patients, families, and team members

Schedule DetailsEight - hour shifts with start times between 7:30 a.m. and 8:00 a.m. depending on unit of coverage. Schedule is 10 days per bi-weekly pay period. Holiday coverage required.
